• Your original order number (found in your receipt)
• Your original packaging slip (if available)

Note: Failure to return all bottles will result in a partial refund. You are responsible for the return shipping cost. Please ship via USPS and provide us with a valid tracking number — failure to provide a tracking number may delay the processing of your refund.
